How to Use This Template
- Fill in your personal details and GP surgery information in the letter below.
- Click the “Print This Letter” button.
- Sign the printed letter and post it or hand-deliver it to your GP surgery.
- You can also submit this request electronically if your surgery accepts email requests.
Important: Under UK GDPR, you have the right of access to your personal data held by your GP surgery, including your Summary Care Record. GP surgeries should respond within one calendar month (this may be extended for complex requests, and the clock starts once your identity has been verified). There is usually no charge, though limited exceptions exist for manifestly unfounded or excessive requests under the Data Protection Act 2018.

I am writing to make a subject access request under Article 15 of the UK General Data Protection Regulation (UK GDPR), as enacted by the Data Protection Act 2018.
I request a copy of my full Summary Care Record (SCR) held by this practice. This includes, but is not limited to:
- Current and past medications
- Known allergies and adverse reactions
- Significant medical history (as recorded on the SCR)
- Any additional information included in my SCR (additional information is included by default unless you have previously opted out — you can change your preferences at your GP surgery)
If there is any information on my SCR that has been enriched with additional data beyond the core SCR dataset, please include this in full.
